In addition to arriving to your first appointment 10 minutes early, and bringing your identification and insurance card, we ask that you complete our new patient forms.

Children (12 years and under)
  • 2 dental wellness exams
  • 2 professional cleanings
  • 2 fluoride treatments
Both plans include one set of bite-wing x-rays + 10% courtesy on all treatment services, including aesthetics and elective.
